Before you arrive
What to bring to a first visit.
A little preparation makes your first appointment smoother. None of it is required to be seen — bring what you can, and we'll help with the rest.
A photo ID
A driver's license, state ID, or passport — anything that confirms your name and date of birth.
Your insurance card
Bring the physical card if you have one. We verify your coverage before the visit so there are no surprises.
A current medication list
Include prescriptions, over-the-counter medicines, vitamins, and supplements — names and doses if you have them.
Records from past care
Recent lab results, imaging, or notes from another physician help us pick up exactly where your last care left off.
Your questions
Write down anything on your mind before you arrive. The visit is yours — nothing is too small to ask about.

We hold dedicated new-patient slots every week. From the first call to the exam room, here's exactly what to expect.

Reach out
Book online or call the office. Tell us a little about what you need and whether you'd prefer an in-person or telehealth visit.
Step two
Complete your intake
We'll send you intake forms to fill out securely before the visit. Prefer paper? Print them at home or arrive a few minutes early.
Step three
Meet your physician
Most new patients are seen within seven days. Your first visit is unhurried — time to talk through your history and build a plan.
Patient portal
Your care, online and secure.
The patient portal is the fastest way to stay connected between visits. From any device you can:
- Message your care team securely
- Request prescription refills
- Review lab results and visit summaries
- Update your contact and insurance details

Intake & forms
Paperwork, done before you arrive.
We'll send your intake forms ahead of time so your first visit can be spent talking, not filling out clipboards. You can complete them digitally through the portal, or print and bring them with you.
Intake covers your medical history, current medications, allergies, and the reason for your visit. If anything is unclear, leave it blank — we'll finish it together.
